Important FormsBelow you will find important forms you may need along the way in your college admissions process. All forms are available in the Guidance Office; however, they may be saved and printed from this site and brought in to your Guidance Counselor.  Remember, if you need a transcript sent to a specific college, see the recommended transcript request deadlines to ensure that your transcript arrives on time.

 

 

 

Academic Profile: Click here to download the FCA High School Academic Profile. This includes information such as where our students rank nationally in ACT testing, AP Test score percentages, which colleges our students are attending after graduation, and much, much more!

 

College Visitation Form: This form is needs to be filled out by any FCA HS student that wishes to visit colleges during the school year. Every student is allotted two Activity Absences for visiting colleges per school-year, but this form must be filled out and signed by the Guidance Department at least three days prior to the student's trip. College Readiness Assessment Form: This form was created to help assist the FCA senior class in beginning the college application process. The assessment form will simply walk students through the basic steps of evaluating college options, majors, financial aid, and letters of recommendation. This form is intended to get students who have not started to think about college in to a mindset of beginning the admissions process and researching the options. Final Transcript Request Form: All seniors need to fill out this form (in April or May of their senior year) in order to request their final transcripts to be sent to the college or university they plan on attending in the Fall. The form can be downloaded by clicking here. Please fill it out in full and return it to the Guidance Office as soon as possible or email it back to your Guidance Counselor. This is a very important step in preparing for college as most colleges and universities require a student's final high school transcript to begin college courses in the Fall. Remember: Final transcripts will not be sent to a student's college without this form (Steps 1 - 5) being completed in full.

 

Recommendation Letter Request Form: Colleges, scholarships, and some employers may require a Recommendation Letter during the application process. Fortunately, you attend a school with teachers and coaches that know you very well and are ready and willing to write you an exceptional letter of recommendation! Click here to download the Recommendation Letter Form you need to give your teachers prior to their writing a letter for you. Make sure to get this in their hands well before the deadline, because they will begin to receive these same forms from your classmates soon!  

  

Transcript Request Form for Current FCA Students: If you are a current FCA HS student and need to have your transcripts sent to a college or university, this form needs to be filled out and turned in to your Guidance Counselor--you may not submit this form electronically. Also, we are not able to take requests over the phone or via email. The Transcript Request Form must be filled out and submitted in person to your Guidance Counselor. Below you will find our recommended Transcript Request Deadlines. In order to process your request and make sure your transcripts arrive at your college of choice in time, we highly recommend submitting the transcript request form by the deadline dates listed below. Click here to download the Transcript Request Form.

Transcript Request Instructions for FCA Alumni/Graduates:
If you are an FCA HS Alum and need your transcripts to be sent to a college, you must contact the FCA HS Registrar via email in order to request your transcripts to be mailed out. Official transcripts can be sent only to education or military institutions or to an employer (not directly to the student). Any transcript handed directly to an FCA Alum will be marked as "unofficial". Important: It can take 3-5 business days to process and send your transcripts. When requesting transcripts, email the Registrar at This e-mail address is being protected from spambots. You need JavaScript enabled to view it and include the following information:

  • Full Name
  • Date of Birth
  • Phone Number
  • Year of FCA HS Graduation
  • College/University/Institution to send transcripts to with address.
    • Including contact name if you would like them to be addressed to any one in specific.
